It feels like we’re living through particularly uncertain and unsettling times, being surrounded by unexpected and unpredictable change to which we react with fear. Our patients face many threats, ranging from the impact of cost-containment on access to care, to protocolized, rigid care pathways, to realization that modern surgery remains plagued with all-too-common major morbidity and mortality. As physicians in anesthesiology, we also face many threats, ranging from restriction and encroachment on our scope of practice from other medical specialties and caregivers, to lack of clear metrics to demonstrate value in a value-based health care system, to competition with caregivers outside of allopathic medicine. And many would argue that, at least in its current form, the electronic health record threatens both patients and physicians. The ASA Monitor explains and explores these threats in each issue, but in this article I argue that a fundamental aspect of our specialty has failed...
